A Material Inspection and Receiving Report, form DD 250, will be prepared and distributed for each shipment made under this Contract in accordance with the U.S. Defense Federal Acquisition Regulation supplement, Appendix F. Questions regarding preparation and distribution of this form may be addressed to the Quality Assurance Representative assigned to the Contractor's plant. As proof of shipment, one (1) signed copy of this inspection document must be supplied to, (insert name of DSS Contracting Officer) _____ and Defense Contract Management Area Operations/Ottawa, P.O. Box 3416, Station D, 365 Laurier Avenue West, Ottawa, Ontario K1P 6L4.
